It’s time to look ahead to a brand-new year of Wildkat athletics and activities! To help our families prepare, Kokomo School Corporation is partnering with Ascension St. Vincent to host a FREE Sports Physical Night.
Whether your student is headed for the court, the field, or the stage, this free screening ensures they are officially cleared for the entire 2026-2027 school year.

Time: 6:00 PM – 8:00 PM
Where: Kokomo High School (2501 S. Berkley Road)
Cost: FREE for all currently enrolled KSC students in grades 5-11
More Than Just Sports: Is Your Student Covered?
A completed IHSAA physical is a requirement for participation in much more than just varsity sports. To ensure safety during high-energy rehearsals and competitions, this clearance is mandatory for:
Athletic Teams & Leagues (All sports, grades 5-12)
Marching Band & Color Guard
Cheerleading
Performing Arts (Certain dance and theater-related extracurriculars)
Middle School Families: This Is Important For You Too!
Attention Grades 5-8: If you are planning to join a new team or try out for a group next year, this is for you! You cannot participate in practices, tryouts, or games without an updated physical on file. Getting this done now for free means you won't have to scramble for a doctor's appointment this summer.
3 Steps to Get Cleared
We use FinalForms to streamline the process. Please complete these steps before you arrive:
Sign Up: Parents/Guardians, visit kokomo-in.finalforms.com.
Register: Enter your student’s medical history and insurance info.
Print & Bring: Print the IHSAA PPE Physical Form from your account.
Important: Students MUST have the printed IHSAA PPE Physical Form in hand to receive their free screening on May 20th.
